• Minimum age of 18 years.
• Possession of a valid student permit.
• High school graduation.

1. Photocopy of your Form 138 (High School Report Card) and Form 137A or High School Diploma or a photocopy of TOR (if college-level/graduate).
2. Barangay Clearance, Police Clearance, or NBI Clearance.
3. Medical Certificate issued by a Public Health Officer.
4. Photocopy of your Birth Certificate (PSA).
5. Six (6) passport-size pictures (chemically printed, white background, wearing a shirt with a collar or a suit, complete with a name tag).
6. Six (6) 1x1 ID pictures (chemically printed, white background, wearing a shirt with a collar or a suit).
7. Driver's License or at least a valid student permit.
8. Long brown envelope.
